Our housing authority serves residents within the
city limits of the City of Waukegan.
We understand that relocating your family
is a challenging task and hopefully this page will
make the transfer of your Section 8 Housing Voucher
a little smoother.
In accordance with HUD Regulation we require
that all Section 8 families relocating to the City
of Waukegan provide the following documentation from
your initial Housing Authority.
Please send a copy of the family's:
- Family Portability Information (HUD Form 52665)
- Copies of Income Verifications
- Housing Choice Voucher (HUD Form 52646)
- Family Report (HUD Form 50058)
- Additional Items- for each family member
- Legal Identification (age 14 and above)*
- Birth Certificates*
- Social Security Cards*
- Local Contact person for the family*
- Local Phone & Address for the family*
- Name of the Initial Housing Authority
- Address & Phone of Initial Housing Authority
- PHA No. for Initial Housing Authority
- Fed. ID No. for Initial Housing Authority
- Administrative Fee of Initial Housing Authority
(* these items may be submitted by the family after
the initial package has arrived)

OUT-GOING PORTABLILITY: If you are a participant
in the Waukegan Housing Authority Section 8 Program
and would like to relocate your family, please submit
a written request to our office. You will be required
to properly terminate the lease and to update your
family's Certification.
